Add a dependent

Note: Depending on your plan, your plan administrator might have turned these features off. If you do not see them, please contact customer support to edit your dependent information.

1. On the Navigation Bar, select the drop-down menu with your name on it.

2. Select Account Settings , then select My Dependents.

3. Select Add a Dependent.

4. On the Add Dependent screen, enter your dependent's Relationship to You, Name, Birth Date, Sex, and Coverage Status.

5. Select an Effective Date for their coverage to start.

6. Select whether they are a Full-time student.

7. Select the Legal checkbox.

8. Do one of the following actions:

   • If your plan includes insurance, select the Continue to Benefit Plan Status button. Select the Health and Dental benefits you want to apply to this dependent. Select Save Changes and then Save.

   • If your plan is a Health EAP plan, select Continue. Then select Save.
