Request Hormonal Health Services

1. On the Navigation Bar, select Care Services and Total Health and Life Services and then Hormonal Health.

2. Review the program description and select Service Request.

3. On the Introduction page, select Start.

4. On the Payment Method on File page, enter your credit card details, and select Next.

   • If you already have credit card details saved on file or are fully sponsored for this program by your organization, you will skip this step. You will not be charged when you enter your payment details during this service request, but it ensures that you have a payment method on file for future transactions.

5. On the Additional Details page, enter any relevant information you would like to share with your assessment nurse, then select Submit.

Next steps to access Hormonal Health Services

1. Once you submit a request for the Hormonal Health program, you will be matched with an assessment nurse to book a free initial assessment.

2. At the assessment, your nurse will discuss details about the Hormonal Health Services, including a summary of your personalized care plan and applicable referrals to other health providers. If you would like to continue with the program, the nurse will enroll you.

3. After your initial nurse assessment, you will be directed to our partner for this program.

4. You will have access to consultations with a nurse practitioner from our partner to evaluate symptoms, discuss test results, and create a personalized care plan.

Please note that even when this program ends, you will still be able to purchase and enrol in additional personalized support to continue your hormonal health journey. It is important that you always have access to the care you need.
