Close your pharmacy account

1. From the Navigation Bar, select View all Care Services and then select Pharmacy.

2. On the Pharmacy Dashboard, select Pharmacy Settings.

3. On the Pharmacy Settings, select Close Account.

4. Provide a reason to close the account and Select reason.

5. Select Close account.

Close your pharmacy account on the mobile app

1. On the Home screen, select Profile Settings.

2. Select the Primary Profile, then select Deactivate Pharmacy Account.

3. Provide a reason to close the account and Select reason.

4. On the Are you sure you would like to deactivate your account? screen, select Deactivate pharmacy account.
