From the Navigation Bar, select View all Care Services and then select Pharmacy.

On the Pharmacy Dashboard, select Pharmacy Settings.

On the Pharmacy Settings, select Close Account.

Provide a reason to close the account and Select reason.

Select Close account.

On the Home screen, select Profile Settings.

Select the Primary Profile, then select Deactivate Pharmacy Account.

Provide a reason to close the account and Select reason.

On the Are you sure you would like to deactivate your account? screen, select Deactivate pharmacy account.
